Save time by filling out paperwork ahead of your appointment and either submitting them electronically or printing them off and bringing them with you!

Submitting Electronically:

-open the file by clicking on the link

-save to your computer

-open file with Adobe Acrobat Reader

-click "fill and sign" and fill out the form

-save file once it has been filled out

-Email saved file to cweiman@cornerstone-fp.com

Printing:

-open the file by clicking on the link

-click "print"

-fill out paperwork and bring with your appointment
